Correct Answer: Option D

Statement 1 is not correct: Bharat Ratna, Padma Vibhushan, Padma Bhushan and Padma Shri are not titles as defined under Article 18(1) of the Constitution. They cannot be used as prefixes or suffixes to the recipient's name.

Statement 2 is not correct: The Padma Awards, instituted in 1954, are announced annually on Republic Day, but there were interruptions in 1978, 1979, and from 1993 to 1997.

Statement 3 is not correct: The Bharat Ratna, India's highest civilian award, recognizes exceptional service/performance. The Prime Minister makes recommendations to the President, and no formal recommendations are necessary. The number of Bharat Ratna awards is restricted to a maximum of three in a year. As of the question date, the Government has conferred the Bharat Ratna on 45 persons.

Hence, all statements are incorrect.
